    What Is Vertebrogenic Pain?

    Vertebrogenic pain is a distinct type of chronic low back pain (CLBP) caused by damage to the vertebral endplates — the cartilage surfaces between vertebrae and spinal discs. This damage can result from disc degeneration or simply the wear and tear that occurs with everyday living.

    The basivertebral nerve (BVN), found within the vertebral body, carries pain signals from these inflamed endplates to the brain. Patients often describe this pain as being in the middle of their low back, made worse by physical activity, prolonged sitting, and bending forward.

    Vertebrogenic pain is diagnosed through clinical evaluation and MRI findings that show Modic changes — signs of inflammation and bone marrow edema in the vertebral endplates.

    How the Intracept Procedure Works

    Diagnosis

    Your physician identifies vertebrogenic pain through clinical evaluation and MRI findings, including Modic changes (bone marrow edema) in the vertebral endplates.

    Access

    Using fluoroscopic guidance, a small cannula is introduced through the pedicle of the affected vertebra to reach the basivertebral nerve.

    Ablation

    A radiofrequency (RF) probe is advanced through the cannula to the basivertebral nerve. Controlled thermal energy ablates the nerve, stopping it from transmitting pain signals.

    Recovery

    The procedure is performed on an outpatient basis. Patients typically return to normal activities within 1–2 weeks, with pain relief improving over the following months.
